All Members and Registered Students must complete this annual declaration.

Members and Registered Students (the “Practitioner”) who sign or otherwise take responsibility for a work product that is addressed by the CBV Institute Practice Standards are subject to a Practice Inspection.

Any work product which is subject to CBV Institute Practice Standards, and that was issued on or after January 1, 2018 is subject to possible selection for inspection. Compliance with the Practice Inspection Program, including completing the declaration below, is mandatory. Please refer to the Mandatory Practice Inspection Policy.

As a Member or Registered Student, please indicate, by completing the below declaration by March 1, 2020, whether you have signed or otherwise taken responsibility for work product(s) which are subject to CBV Institute Practice Standards during the period January 1, 2018 to December 31, 2019 (the “Inspection Period”)?

Note that a Yes answer identifies you as a Practitioner who is subject to Practice Inspection by CBV Institute for that period.

For additional information about the Practice Inspection program, please refer to the Mandatory Practice Inspection Policy.

For any questions regarding Practice Inspections and work product that is potentially subject to Practice Inspection, please contact Catalina Miranda, Vice President, Regulatory and Standards, at catalina.miranda@cbvinstitute.com.
